Begin by providing basic demographic information to let us know who you are and how to contact you. Later on in the booking process we will need copies of the order and medical insurance. This helps us to ensure your personalized healthcare needs are met.​

Location
Let us know where you’d like your concierge service to take place—whether you’re at home, on the go or at the office and we’ll come to you with all the necessary equipment. For on-demand services outside the state of FL and GA- we require a notice in advance to ensure we can book a flight to get you the care you need when you need it.

 

Preferred Lab Facility

Some insurance providers require specimens to be taken to a specific lab for in order for the testing to be covered by the insurance. If you have a preference of which lab you would like your specimens tested, please let us know. Select the lab facility where you’d like your specimens to be sent for analysis.

Upload Lab Order
Your trusted partner in Healthcare, we're here for you. If your doctor has provided you with lab orders, you can easily upload them directly through the booking system. Simply take a picture or scan your lab order and attach it to your booking form. You may also text a picture or email a copy of your lab order. Simplicity is the ultimate sophistication. You may also request for your doctor's office fax the orders directly to us; we will reach out to you to confirm receipt and assist you with scheduling.

Pricing and Quotes

Affordable healthcare, without compromise. Our company uses prestige pricing which helps ensure all quotes are tailored to your specific needs. Although deposits are not required to book an appointment, you will still need to complete the booking form in order to obtain a quote for services. Please be sure to provide a copy of your order along with your address to ensure you receive your tailor-made quote.
